Alumni Transcript Request Form

Request A Transcript:
Requests for alumni transcripts will only be accepted in writing. Telephone, fax, and walk-in requests for transcripts will not be accepted. No exceptions!

Please fill out the form and mail it to us with a $5.00 money order (no cash and no personal checks). Note there is a 2-3 week processing time for all transcripts.

For further information you may call Ms. Barrett at 718-281-3727  between the hours of 8:30 am and 1:30 pm Monday-Friday.
